All students entering Our Lady of Sorrows School must have an up-to date physical examination and immunizations prior to attending school. No student will be admitted to class without satisfying these requirements. It is the parent or guardian’s responsibility to provide all information necessary for school entrance.
If entering Our Lady of Sorrows School from another school in New Jersey, you must have a copy of your child’s most recent physical exam including immunization record. All starred (*) items must be completed. The State of New Jersey requires a physical exam within one year of the date of entry into school. OLS also requests a physical in third grade.
If entering from out of state the student must have Health Assessment completed by a physician, including the immunization record. All items must be completed and the exam must have been done within a year of entry into OLS.
If entering OLS from out of the country, the student must have a Health Assessment completed by a New Jersey Physician, including immunization record and all starred items, within one year prior to entry into OLS. If the student is entering from a high risk country as defined by the CDC, a PPD Tuberculosis test is required.
